Transition Consult

For the family that says, "I need to understand what's going on and what to do next." One focused session, in person or by video. A new diagnosis, a transfer decision, a bill nobody can explain, or the question "should we be getting help with this?" β€” this is where you start.

  • We go through the whole situation together
  • Plain-language explanation of what's happening and what the options are
  • Records, patient portal and paperwork sorted out
  • The questions to ask at the next appointment, written down
  • A one-page plan: what's blocking things, who to call, in what order
  • Counts toward a package if you decide to keep going within two weeks

Continuity Plans

For the family that says, "We need someone in our corner, month after month." Chronic conditions, long-distance families, a parent with more appointments than anyone can track. Month to month, no long contract, and it works for one parent or both. Each plan includes a set number of hours every month; on the call we match the plan to how much is going on.

Essential

I keep watch and coach you. You make the calls.

  • Monthly check-in call with you or your parent
  • I watch the patient portal, test results and upcoming appointments
  • A heads-up before each appointment with the two or three questions worth asking
  • Same-week response when something comes up
  • Records, referrals and portal problems sorted out as they come up
  • A one-page care summary kept current β€” medications, providers, upcoming dates, who to call
  • A short family update after each check-in
  • By phone, video and portal β€” an in-person visit is billed as additional hours

Full Support

I make the calls and I go with you. In-person appointments are included β€” a typical month fits two or three. Everything in Essential, plus:

  • I attend appointments with you or your parent, take the notes, and send the family a plain-language update afterward
  • I speak directly with the clinic, the nurses, the case manager and the prescriber β€” you are no longer the go-between
  • After each visit I follow up until what was ordered actually happens: referrals, tests, prescriptions, records
  • Specialists coordinated so they're working from the same information
  • Insurance, prior-authorization and billing problems worked on your behalf, not just explained
  • Care conferences and family meetings β€” I prepare you, and I'm in the room
  • If a hospital stay or a big change happens, I'm the first call

Also Available

A few situations don't fit neatly into the three options above. These are the add-ons we can talk through on the call.

  • Extended Support Block β€” a set block of hours over about two weeks, for a bigger stretch: several appointments in a row, a facility search, an insurance appeal
  • Getting Started β€” a one-time onboarding for a heavy start, such as two parents, two states, or portals and records that were never set up
  • Additional hours β€” billed by the hour in 15-minute increments, always approved with you in advance
  • In-person visits β€” included in the Consult, the Package, the Block and Full Support: the time at the visit counts toward the plan's hours, and travel time is not counted within the Milwaukee-area service ring (beyond it, travel counts at half). Mileage at the standard IRS rate. Essential is phone, video and portal; a visit there is billed as additional hours.
  • Se habla espaΓ±ol β€” every service is available in Spanish

A note on fees: The Consult and the Package are flat fees. Continuity Plans are a flat monthly fee. You'll know the exact number before anything starts, and there are no surprises after. CarePath Advocate is private pay and independent β€” I don't bill insurance, which is what lets me answer only to you. We'll talk through your situation and the right fit during your free, no-obligation 30-minute call.
